If you and your partner are having trouble conceiving, a semen analysis is often the first and most informative test to perform. This simple, noninvasive laboratory evaluation measures the quantity and quality of a man’s sperm and can determine whether there is a male factor contributing to infertility—a factor present in approximately 60% of all couples experiencing fertility challenges. At Maze Men’s Health, under the direction of Michael A. Werner, MD, a board-certified urologist and fellowship-trained specialist in male infertility, we provide comprehensive and highly accurate semen analyses for patients throughout Westchester, New York City, Connecticut, and New Jersey.

Semen Analysis Overview

A semen analysis provides a window into a man’s reproductive health. It evaluates both sperm production and the fluid that carries the sperm. When performed correctly, it can yield a remarkable amount of diagnostic information about hormone function, potential blockages, infections, or genetic causes of low sperm counts. Since a male factor plays a role in 60% of infertility cases, the semen analysis should always be part of an initial fertility workup.

Approximately 85% of couples will conceive within one year of regular, unprotected intercourse. However, if conception does not occur after twelve months—or after six months if the female partner is thirty-five or older—it’s time to begin an evaluation. Because sperm take about three months to develop and mature, results from a semen analysis reflect what was happening in the body months earlier. This three-month production cycle means that any lifestyle changes—positive or negative—can take up to a full season to affect results. It also explains why it’s important to perform more than one semen analysis to establish an accurate baseline and account for natural fluctuations in sperm count and motility.

What We Measure and Why It Matters

During the semen analysis, we evaluate multiple parameters that together provide a complete picture of fertility potential. We first measure semen volume, which indicates the total amount of ejaculate. A lower-than-expected volume can suggest incomplete collection, hormonal issues, or a blockage in the reproductive ducts. We then assess sperm concentration, which measures how many sperm are present in each milliliter of fluid. Both numbers are crucial: a man may produce a large volume of semen but have a low sperm concentration—or the opposite—and both can affect overall fertility.

We also evaluate sperm motility, which measures the percentage of sperm that are moving, and forward progression, which describes the quality and direction of their movement. Even if sperm are present, they must swim efficiently to reach and fertilize an egg. Morphology refers to the shape of the sperm. Sperm must have a normal head and tail structure to penetrate the egg’s outer layer successfully. Another key metric is the total motile count, which combines volume, concentration, and motility to determine how many moving sperm are present in the entire ejaculate.

Each of these parameters helps determine the likelihood of natural conception or the potential need for assisted reproductive techniques. While certain values are associated with higher fertility rates, it is important to remember that there is no single “perfect number.” Some men with lower counts may still conceive easily, while others with strong results may still face challenges. What matters most is the pattern of findings and the context of the couple’s overall fertility evaluation.

Specialized Testing for More Complex Cases

When a semen analysis reveals abnormalities or when results are on the lower end of normal, Maze offers several advanced tests to refine the diagnosis. A spun-pellet analysis involves centrifuging the semen sample to concentrate any sperm that may be present in very low numbers. Our andrologists then examine this concentrated sample under high magnification to identify even a single viable sperm, which can be cryopreserved for future use in IVF or ICSI.

Viability testing helps differentiate living but immobile sperm from dead sperm. This distinction is essential, as immotile but viable sperm can still be used in assisted reproductive procedures. We also perform fructose testing, which can help determine whether the issue is related to sperm production or a blockage that prevents sperm from entering the semen. Another specialized test, post-ejaculatory urinalysis, helps identify retrograde ejaculation—a condition in which sperm travel backward into the bladder instead of exiting through the urethra.

Each of these tests provides valuable insight that can change the course of treatment and improve a couple’s chances of conception.

How the Results Influence Treatment

A comprehensive semen analysis not only identifies potential causes of infertility but also guides the most effective treatment plan. More than half of men with abnormal results have a reversible condition such as a varicocele, hormonal imbalance, an infection, or an obstruction in the reproductive tract. Correcting these problems—whether through microsurgical procedures, medications, or lifestyle changes—often leads to measurable improvements in sperm count, motility, and morphology within a few months.

For men whose semen parameters remain suboptimal, advanced reproductive technologies may offer the best chance of success. At Maze, we collaborate closely with reproductive endocrinologists to provide seamless integration between diagnostics and treatment. Depending on the results, couples may be advised to pursue intrauterine insemination (IUI), in-vitro fertilization (IVF), or intracytoplasmic sperm injection (ICSI). For men with very low or zero sperm counts, Maze offers exclusive options such as Extended Sperm Search & Microfreeze (ESSM) and Grid Needle Sperm Extraction & Microfreeze (GN-SEM)—innovative, minimally invasive procedures developed by Dr. Werner that have helped many couples conceive when other methods have failed.

Preparing for the Test

Proper preparation ensures the most accurate results. Men should abstain from ejaculation for 48 hours before the test, as longer periods can increase count but reduce motility, and shorter intervals may lower count but improve quality. Semen samples can be collected at home or in our private, comfortable collection rooms. If collected off site, the sample should be kept at body temperature and delivered to the lab within one hour.

We understand that producing a specimen on demand can feel awkward or stressful. Our staff is compassionate and discreet, and we offer accommodations to make the process as comfortable as possible. For those who prefer, partners may accompany patients into the collection room. Men who experience difficulty achieving an erection or ejaculation may be prescribed a short-acting oral medication such as Viagra or Cialis, which does not affect sperm quality. For individuals who do not masturbate for personal or religious reasons, Maze provides non-spermicidal collection condoms for use during intercourse, allowing for accurate testing without compromising beliefs or comfort.
