Penile Rehabilitation

Treatment for prostate cancer (or another illness, surgery, or trauma) doesn’t mean you have to suffer with erectile dysfunction. Active and immediate rehabilitation will maximize your long-term sexual functioning.

What is Penile Rehabilitation?

Penile rehabilitation is a medical and therapeutic program designed to restore penile health, protect erectile function, and support sexual recovery after prostate cancer treatment, surgery, trauma, illness, or long periods of erectile dysfunction. It is an emerging field in sexual medicine, and at Maze, we help men proactively recover — not wait and hope things improve on their own.

Why Penile Rehab Matters

Many men face erectile dysfunction, changes in sexual sensation, loss of penile size, and emotional or relationship challenges following prostate cancer treatment. A structured penile rehabilitation plan:

  • Supports physical healing and tissue health
  • Maintains sexual activity and intimacy
  • Reduces long-term damage from disuse
  • Improves recovery outcomes and emotional wellbeing

The earlier rehab starts — ideally before treatment — the better the results.

Program Goals

A penile rehab program aims to:

  • Restore sexual function as soon as possible
  • Use the least intervention needed to achieve results
  • Prevent long-term fibrosis, atrophy, and penile shortening
  • Teach men to manage their sexual health for life

When to Begin

Ideally, rehabilitation begins before prostate cancer treatment or surgery, but it should start as soon as possible afterward.
Even if months or years have passed, it is never too late to benefit from penile rehabilitation.

What to Expect at Your First Visit

Your first appointment includes:

  • A comprehensive sexual health assessment
  • Questionnaires on mood, sleep, lifestyle, relationships, and function
  • A session with a sex counselor
  • Evaluation by specialists in erectile dysfunction
  • Possible diagnostic testing and immediate treatment initiation

Partners are encouraged to attend, as sexual recovery is often a shared process.

What Your Treatment Plan May Include

Treatment is personalized based on your history, symptoms, and goals. Components may include:

Medical Therapies

  • PDE5 inhibitors such as Cialis (often used pre- and post-operatively)
  • Penile injections for men who need stronger, reliable erections

These therapies protect blood vessels, improve tissue oxygenation, and help maintain erections.

Therapeutic & Physical Treatments

  • Penile exercises and tissue stimulation
  • Vacuum erection devices to increase blood flow and combat atrophy
  • Vibration therapy to stimulate nerves and restore sensation
  • Stretching therapy to address lost length or curvature when needed

Regular stimulation — including masturbation, partner intimacy, and therapy-guided sexual activity — is encouraged because a penis that is used and exercised heals better.

Expected Recovery & Success

Outcomes vary because men begin rehab with different levels of impairment, but research shows:

  • Men who start rehab early recover significantly better
  • A “wait and see” approach leads to poorer long-term results
  • Consistent effort dramatically increases the likelihood of returning to spontaneous erections
