Feeling Flat, Foggy, or Just “Off”? Low Testosterone Could Be the Culprit

What Is Low Testosterone (Low T)?
Testosterone is the primary male hormone, and it does a lot more than build muscle and fuel your sex drive. It influences energy, focus, confidence, and, yes, your mood. As men age, testosterone naturally declines, and some men dip low enough to develop real symptoms.
Clinically, “Low T” (hypogonadism) means two things together: a consistently low testosterone level on blood testing plus symptoms (American Urological Association Guideline, 2018). Most guidelines use a total testosterone below roughly 300 ng/dL, measured on two separate early-morning blood samples, as the cutoff (American Urological Association, 2018; Anawalt et al., JAMA, 2026).
Common Low T Symptoms to Watch For
Low testosterone shows up in more ways than you might expect. Classic signs include:
- Low sex drive and fewer spontaneous erections
- Fatigue and low energy
- Depressed mood, irritability, or feeling “flat”
- Brain fog, poor concentration, and low motivation
- Loss of muscle mass and increased body fat
Those mood and mental symptoms are the ones men tend to ignore — but they matter (American Urological Association, 2018; Bhasin et al., Endocrine Society Guideline, 2018).
The Low T and Mood Connection: It’s Real
Here’s where it gets interesting. Many symptoms of Low T overlap with the symptoms of depression — low mood, fatigue, loss of enjoyment, and trouble concentrating (Hauger et al., Reviews in Endocrine & Metabolic Disorders, 2022).
And the research backs up the link. In a large UK Biobank study of more than 133,000 men, both low total and low free testosterone were associated with a higher risk of developing depression, with the effect strongest in middle-aged men (He et al., Journal of Affective Disorders, 2026).
Can Testosterone Actually Improve Your Mood?
For the right man, yes. Testosterone replacement therapy (TRT) has been shown to give mood a meaningful lift:
- In the landmark TRAVERSE trial, men with Low T who took testosterone had modest but significant improvements in mood and energy compared to placebo (Bhasin et al., Journal of Clinical Endocrinology & Metabolism, 2024).
- A major meta-analysis of 27 randomized trials found that testosterone treatment significantly reduced depressive symptoms compared to placebo (Walther et al., JAMA Psychiatry, 2019).
The honest caveat: TRT is not a cure-all. The biggest benefits are seen in men who truly have low testosterone, and testosterone is not a replacement for proper treatment of major clinical depression (Bhasin et al., Endocrine Society Guideline, 2018). That’s exactly why personalized evaluation matters.
